Physical Examination

Athletes must have a current Sports Qualifying Physical Exam on file in the Activities Office in order to participate. Physicals are good for 3 years from the exact date of the last physical.  Physicals expiring between March 12, 2020 and the end of the 2020-2021 competition year will be granted an extra year of clearance.

Once you have your physical completed by a doctor, you can turn it into the Athletic office or you can scan it and submit the completed Physical Forms online.
